Do you have fuel getting the cylinder? How about spark? Is the air filter clogged? Check those things first. Check the coil as clinton said....

what engine? either way check the vaccum line going to the fuel regulator. it should be somewhere on the fuel line.

correct. only the one close to the tailgate im sure you could do the one in the front if you moved the filler neck outta the way while you drilled
those are clearences for the fuel filler neck. the beds have two causes they never changed the stamp for the beds with no rear tank.
